1. Download and Print Out the form

Choose if you want a Credit Card or a Prepaid Card
2. Fill and Sign it

Check the Requirements, choose your additional Services and Sign it
3. Sent it

Put the signed form into an envelop with a copy of your Swiss Passport, Swiss ID or residency permiss (B, C) and sent it to:

4. Use

After approval of Cornèrcard, you are going to receive your Made in Italy Card ready to be accepted by MasterCard network.
